ABSTRACT

Inflammatory bowel disease (IBD) is a problem that directly affects the quality of life of patients suffering from this condition. Monitoring the serum level of infliximab (IFX) (TDM) is an important tool for guiding therapeutic decisions in IBD patients. The purpose of this study was to determine the significance of quantitatively measuring the serum level of IFX (TDM) and antibody to IFX (ATI). Methods and materials: Prospective observational study involving 40 IBD patients on IFX therapy, including 14 Proactive (week 06 of the induction phase) and 26 Reactive (maintenance phase). Immediately prior to the infusion, blood samples were drawn and measured using a Bulhlmann rapid test instrument. Serum concentrations of IFX were categorized as supratherapeutic (>7.0 micrograms/ml), therapeutic (between 3.0 and 7.0 micrograms/ml), and subtherapeutic (3.0 micrograms/ml). When the serum concentration of IFX was 3 mcg/ml (subtherapeutic), the ATI was measured. 25 patients with CD and 15 patients with UC were evaluated. Only three of the twenty patients with subtherapeutic serum levels had a positive ATI, and both were reactive; two had CD and one had UC. There was a statistically significant difference between reactive and proactive patients with respect to levels of CRP (p = 0.042), with proactive DNS patients suffering greater alterations in CRP and albumin. ABSTRACT

Objective: To evaluate the application of proactive pro-drug therapy (TDM) at week six in users of infliximab therapy in ulcerative colitis patients and to analyze the need for further disease optimization. Method: This is a retrospective analysis that will be carried out simultaneously at the Hospital de Clínicas de Passo Fundo and at the Endoclin Diagnostic Center in the city of Passo Fundo, with secondary data collection between January 2020 and May 2022. The sample included patients from both sexes, regardless of age, who are being followed up in the services mentioned above, by signing the informed Free and Clarified Consent Term. Results: 63.2% of patients required optimization of their treatment based on the serum level assessment at week six. Conclusion: Proactive TDM performed at week six benefits patients in order to complete indications for treatment to avoid lack of drug response and complications from the disease. (AU)

ABSTRACT

SUMMARY: Cisplatin (Cis) is an important chemotherapeutic agent used in cancer treatment. Males exposed to Cis were reported to exhibit testicular toxicity. Cis-induced testicular toxicity is mediated by oxidative stress, inflammation, testosterone inhibition and apoptosis. Accordingly, this study was conducted to evaluate the potential protective roles of infliximab (IFX), which is an anti- TNF-a agent, and of white tea (Camellia sinensis), which is known to possess antioxidant, anti-apoptotic, and anti-inflammatory effects, against Cis-induced testicular toxicity in rats. Rats were randomly assigned into five groups as follows: control group, Cisplatin (7 mg/kg) treatment group, Cisplatin (7 mg/kg) + infliximab (7 mg/kg) treatment group, cisplatin + white tea (WT) treatment group, and Cisplatin+ WT+IFX combined treatment group. In the present study, Cis exposure reduced the sperm count. It also increased testicular oxidative stress as well as the levels of inflammatory and apoptotic markers. Histopathological assays supported the biochemical findings. Treatment with IFX and/or WT restored testicular histology, preserved spermatogenesis, suppressed oxidative stress and apoptosis, and significantly ameliorated Cis-induced damage. It was concluded that white tea and infliximab could potentially serve as therapeutic options for the protection of testicular tissue against the harmful effects of Cis.


El cisplatino (Cis) es un importante agente quimioterapéutico utilizado en el tratamiento del cáncer. Se informó que los hombres expuestos a Cis exhibieron toxicidad testicular. La toxicidad testicular inducida por Cis está mediada por el estrés oxidativo, la inflamación, la inhibición de la testosterona y la apoptosis. En consecuencia, este estudio se realizó para evaluar las posibles funciones protectoras de infliximab (IFX), un agente anti-TNF-α, y del té blanco (Camellia sinensis), conocido por sus propiedades antioxidantes, antiapoptóticas y anti-TNF-α -efectos inflamatorios, contra la toxicidad testicular inducida por Cis en ratas. Cinco grupos de ratas se asignaron al azar de la siguiente manera: grupo control, grupo de tratamiento con cisplatino (7 mg/ kg), grupo de tratamiento con cisplatino (7 mg/kg) + infliximab (7 mg/kg), grupo de tratamiento con cisplatino + té blanco (WT), y grupo de tratamiento combinado Cisplatino+ WT+IFX. En el presente estudio, la exposición a Cis redujo el conteo de espermatozoides. También aumentó el estrés oxidativo testicular, así como los niveles de marcadores inflamatorios y apoptóticos. Los ensayos histopatológicos respaldaron los hallazgos bioquímicos. El tratamiento con IFX y/o WT restauró la histología testicular, preservó la espermatogénesis, suprimió el estrés oxidativo y la apoptosis, y mejoró significativamente el daño inducido por Cis. Se concluyó que el té blanco y el infliximab podrían potencialmente servir como opciones terapéuticas para la protección del tejido testicular contra los efectos nocivos de Cis.

Rationale: the use of anti-TNFα therapy, such as Infliximab (IFX), in patients with Chron's disease (CD) can lead to changes in body composition. Objective: to evaluate the body composition and functional capacity of patients with CD. Method: Cross-sectional study with patients with CD in clinical remission using IFX. For anthropometric evaluation, it was measured: weight, height and waist circumference; functional capacity by the hand grip strength test and body composition by bioelectric impedance. After that, the fat-free mass index and body fat index were calculated. Continouns variables were analyzed by Pearson or Spearman coefficient. For the multiple linear regression model, the time of use of IFX was used as a dependent variable and waist circumference, fat-free mass index, phase angle and handgrip strength were used as independent variables. Results: forty-three patients were evaluated, with an average of 43.1± 13.5 years of age. Of the total, 44.2% were overweight and 44.2% had increased waist circumference, 58.1% were classified with high to very high fat-free mass, 30.2% were below the adequacy parameter for fat-free mass index and 11.6% had reduced functional capacity. Conclusion: overweight, as well as increased waist circumference and body fat, is common in CD patients in clinical remission using Infliximab. Most patients had preserved functional capacity, however there wasn't association with time of use of IFX.


Introdução: o uso de terapia anti-TNFα, como o Infliximabe (IFX), em pacientes com doença de Chron (DC) pode levar a alterações na composição corporal. Objetivo: avaliar a composição corporal e capacidade funcional de pacientes com DC. Metodologia: estudo transversal com pacientes com DC em remissão clínica, em uso de IFX. Foi realizada avaliação antropométrica (peso, estatura e circunferência da cintura); avaliação de capacidade funcional (teste de força de pressão manual); e avaliação de composição corporal (impedância bioelétrica) com cálculo do índice de massa livre de gordura e índice de gordura corporal. Variáveis contínuas foram analisadas pelo coeficiente de correlação de Pearson ou Spearman. Para o modelo de regressão linear múltipla, o tempo de uso do IFX foi utilizado como variável dependente e circunferência da cintura, índice de massa magra, ângulo de fase e força de preensão manual foram utilizados como variáveis independentes. Resultados: foram avaliados 43 pacientes, com média de idade de 43,1± 13,5 anos. 44,2% estavam acima do peso e 44,2% possuíam circunferência da cintura aumentada, 58,1% foram classificados com índice de gordura corporal de alto a muito alto, 30,2% estavam abaixo do parâmetro de adequação para o índice de massa livre de gordura e 11,6% apresentaram capacidade funcional reduzida. Conclusão: o excesso de peso e o aumento de circunferência da cintura e gordura corporal é frequente em pacientes com DC em remissão clínica e em uso de IFX. A maioria dos pacientes apresentou capacidade funcional preservada, porém não houve associação com o tempo de uso do IFX.

Vedolizumabe e anticorpos anti-TNF-α (infliximabe, adalimumabe, certolizumabe pegol). Indicação: Tratamento pessoas com doença de Crohn com falha a um agente biológico anti-TNF-α em tratamento prévio. Pergunta: Para adultos com doença de Crohn moderada a grave com falha terapêutica para anticorpos monoclonais anti-TNF-α, em tratamento de segunda linha, Vedolizumabe tem efeitos superiores aos anti-TNF-α para induzir e manter a remissão da doença? Objetivo: Investigar a eficácia e segurança do vedolizumabe, comparado aos agentes anti-TNF-α (infliximabe, adalimumabe, certolizumabe pegol), na indução e manutenção da remissão em pacientes refratários aos anti-TNF-α com doença de Crohn moderada a grave. Métodos: Revisão rápida de revisões sistemáticas. Levantamento bibliográfico foi realizado nas bases de dados PUBMED, EMBASE, SCOPUS, BVS, Cochrane Library e em registros de revisões sistemáticas e ensaios clínicos. Seguiu estratégias de buscas predefinidas. Foi feita avaliação da qualidade metodológica dos estudos incluídos através da ferramenta AMSTAR-2 (Assessing the Methodological Quality of Systematic Reviews Version) Resultados: Foi selecionada uma revisão sistemática, que atendida aos critérios de elegibilidade, mas nenhum ensaio clínico foi escolhido, pois não atendiam aos critérios de inclusão. Conclusão: Adalimumabe, disponível no Sistema Único de Saúde, é mais eficaz que vedolizumabe para induzir remissão clínica em pacientes tratados previamente com biológicos. Vedolizumabe não é mais eficaz que placebo para induzir remissão clínica. Vedolizumabe e adalimumabe são similares entre si e são mais eficazes que placebo para manter a remissão clínica. Não foram encontradas evidências comparando vedolizumabe a infliximabe ou certolizumabe pegol

Question: For adults with moderate to severe Crohn disease with treatment failure for anti-TNF-α monoclonal antibodies, in second-line treatment, does vedolizumab have superior effects to anti-TNF-α in inducing and maintaining disease remission? Objective: To investigate the efficacy and safety of vedolizumab, compared to anti-TNF-α agents (infliximab, adalimumab, certolizumab pegol), in the induction and maintenance of remission in moderate to severe Crohn disease refractory to anti-TNF-α previous treatment. Methods: Rapid review of systematic reviews. A bibliographic search was done in the PUBMED, EMBASE, SCOPUS, BVS, Cochrane Library databases and in registries of systematic reviews and clinical trials. The search has followed predefined strategies. The methodological quality of the included studies was evaluated using the AMSTAR-2 tool (Assessing the Methodological Quality of Systematic Reviews Version 2). Results: A systematic review was selected, which met the eligibility criteria, but no clinical trials were chosen as they did not meet the inclusion criteria. Conclusion: Adalimumab, available in the Brazilian Public Health System, is more effective than vedolizumab to induce clinical remission in patients previously treated with biologics. Vedolizumab is no more effective than placebo in inducing clinical remission. Vedolizumab and adalimumab are similar to each other and are more effective than placebo in maintaining clinical remission. No evidence was found comparing vedolizumab to infliximab or certolizumab pegol

Study Aim: The aim of the present study was to compare in real life the characteristics of treatment with infliximab according to the presence or absence of anoperineal involvement in Crohn's disease. Methods: We performed a single-center, prospective, non-interventional study, on patients with Crohn's disease in remission who had been treated with infliximab for at least 1 year. Patients with poor treatment compliance, on antibiotics, or those with a stoma were excluded. Results: We included 52 patients in this study: 34 with anoperineal lesions with or without luminal lesions, and 18 with luminal lesions only. Patients with anoperineal lesions were more likely to have undergone surgery (70.6% versus 38.9%, p = 0.027), had a shorter median time to infliximab treatment initiation (0.5 versus 5.5 years, p = 0.005), a higher mean dose of infliximab (6.6 versus 5.1 mg/kg, p = 0.015), and were more likely to receive combination treatments including infliximab (52.9% versus 11.1%, p = 0.008) than patients with luminal involvement only. Conclusions In our study, infliximab treatment was initiated more quickly, at higher doses, and more in combination therapy for anoperineal Crohn's disease than for luminal damage alone. Additional studies are required to confirm this finding and to assess the tolerance of this treatment throughout patient management. (AU)

ABSTRACT

A doença de Crohn é uma patologia caracterizada pela inflamação transmural do trato gastrointestinal, compondo o espectro das doenças inflamatórias intestinais. Nos casos mais graves, dispõe de tratamento com uso de agentes biológicos e imunomoduladores que podem à reativação ou exacerbação de doenças infecciosas preexistentes. Este relato de caso trata de uma paciente do sexo feminino de 24 anos, diagnosticada com Doença de Crohn há 10 anos, evoluindo com necessidade de tratamento com infliximab e, após período de menos de 1 ano, apresentou odinofagia progressiva, dor abdominal e diarreia, além de perda ponderal, sudorese noturna e febre diária. Tomografia computadorizada de tórax evidenciou árvore em brotamento, sendo confirmado diagnóstico de tuberculose pulmonar pelo Teste Rápido Molecular no escarro e provável tuberculose laríngea e intestinal.


Crohn's disease is a pathology characterized by transmural inflammation of the gastrointestinal tract, comprising the spectrum of Inflammatory Bowel Diseases. In the most severe cases, treatment using biological agents and immunomodulators may be available, which can lead to the reactivation or exacerbation of preexisting infectious diseases. This case report is about a 24-year-old female patient, diagnosed with Crohn's disease 10 years ago, evolving in need of treatment with Infliximab and, after a period of less than 1 year, she presented progressive odynophagia, abdominal pain and diarrhea, in addition to weight loss, night sweats and daily fever. Chest computer tomography showed a tree in bud, and the diagnosis of pulmonary tuberculosis was confirmed by the Rapid Molecular Test in the sputum and probable laryngeal and intestinal tuberculosis.

ABSTRACT

Objective: To investigate the efficacy and safety of infliximab (IFX) therapy for children with Kawasaki disease. Methods: Sixty-eight children with Kawasaki disease who received IFX therapy in Children's Hospital of Fudan University from January 2014 to April 2021 were enrolled. The indications for IFX administration, changes in laboratory parameters before and after IFX administration, response rate, drug adverse events and complications and outcomes of coronary artery aneurysms (CAA) were retrospectively analyzed. Comparisons between groups were performed with unpaired Student t test or Mann-Whitney U test or chi-square test. Results: Among 68 children with Kawasaki disease, 52 (76%) were males and 16 (24%) were females. The age of onset was 2.1 (0.5, 3.8) years. IFX was administered to: (1) 35 children (51%) with persistent fever who did not respond to intravenous immunoglobulin (IVIG) or steroids, 28 of the 35 children (80%) developed CAA before IFX therapy; (2) 32 children (47%) with continuous progression of CAA; (3) 1 child with persistent arthritis. In all cases, IFX was administered as an additional treatment (the time from the onset of illness to IFX therapy was 21 (15, 30) days) which consisted of second line therapy in 20 (29%), third line therapy in 20 (29%), and fourth (or more) line therapy in 28 (41%). C-reactive protein (8 (4, 15) vs. 16 (8, 43) mg/L, Z=-3.38, P=0.001), serum amyloid protein A (17 (10, 42) vs. 88 (11, 327) mg/L, Z=-2.36, P=0.018) and the percentage of neutrophils (0.39±0.20 vs. 0.49±0.21, t=2.63, P=0.010) decreased significantly after IFX administration. Fourteen children (21%) did not respond to IFX and received additional therapies mainly including steroids and cyclophosphamide. There was no significant difference in gender, age at IFX administration, time from the onset of illness to IFX administration, the maximum coronary Z value before IFX administration, and the incidence of systemic aneurysms between IFX-sensitive group and IFX-resistant group (all P>0.05). Infections occurred in 11 cases (16%) after IFX administration, including respiratory tract, digestive tract, urinary tract, skin and oral infections. One case had Calmette-Guérin bacillus-related adverse reactions 2 months after IFX administration. All of these adverse events were cured successfully. One child died of CAA rupture, 6 children were lost to follow up, the remaining 61 children were followed up for 6 (4, 15) months. No CAA occurred in 7 children before and after IFX treatment, while CAA occurred in 54 children before IFX treatment. CAA regressed in 23 (43%) children at the last follow-up, and the diameter of coronary artery recovered to normal in 10 children. Conclusion: IFX is an effective and safe therapeutic choice for children with Kawasaki disease who are refractory to IVIG or steroids therapy or with continuous progression of CAA.

ABSTRACT BACKGROUND: A healthy diet is recommended for patients with Crohn's disease (CD) in remission. OBJECTIVE: To evaluate the diet quality of patients with CD. METHODS: Cross-sectional study with patients with CD and clinical remission using the biological agent infliximab. The diet quality was assessed using the Diet Quality Index-Revised (DQI-R). DQI-R was calculated based on 24-hour dietary recalls (24HR), being classified as "inadequate diet" (≤40 points), "diet requiring modifications" (41 to 64 points) and "healthy diet" (≥65 points). Weight, height and waist circumference (WC) of patients were assessed. For comparison between groups, Student's t-test or Mann-Whitney was used. For correlation between continuous variables, Pearson or Spearman coefficient was used. Values of P<0.05 indicated statistical significance. RESULTS: A total of 43 patients participated in the study. The final DQI-R score was 49.1 points - "diet requiring modifications". No patient received the classification of "healthy diet" (maximum score =59.7), 55.8% presented "diet requiring modifications" and 44.2% "inadequate diet". When comparing the "inadequate diet" and "diet requiring modifications" groups, a lower mean age was observed in the "inadequate diet" group (37.6±14.8 versus 47.4±10.5 y, P=0.02). It was found that 44.2% of the patients were overweight (body mass index [BMI] ≥25 kg/m²) and had increased WC (women: WC ≥80 cm and men: WC ≥94 cm). A positive correlation was found between the final DQI-R score and BMI (P=0.046; r=0.346). CONCLUSION: Patients with CD in clinical remission using infliximab are not adopting a diet considered healthy, which points to the need for an individualized nutritional approach.


RESUMO CONTEXTO: É recomendado alimentação saudável para pacientes com doença de Crohn (DC) em remissão. OBJETIVO: Avaliar a qualidade da dieta de pacientes com DC. MÉTODOS: Estudo transversal com pacientes com DC em remissão clínica e em uso do imunobiológico infliximabe. A qualidade da dieta foi avaliada pelo índice de qualidade da dieta revisado (IQD-R). O IQD-R foi calculado a partir do recordatório 24 horas, sendo classificado em "dieta inadequada" (≤40 pontos), "dieta que requer modificações" (41 a 64 pontos) e "dieta saudável" (≥65 pontos). Os pacientes foram avaliados quanto ao peso, altura e circunferência da cintura (CC). Para comparação entre grupos foi utilizado o test-t de Student ou Mann-Whitney. Para correlação entre variáveis contínuas foi utilizado o coeficiente de Pearson ou Spearman. Valores de P<0,05 indicaram significância estatística. RESULTADOS: Participaram do estudo 43 pacientes. A pontuação final do IQD-R foi de 49,1 pontos - "dieta que requer modificações". Nenhum paciente recebeu a classificação de "dieta saudável" (pontuação máxima =59,7), 55,8% apresentaram "dieta que requer modificações" e 44,2% "dieta inadequada". Ao comparar os grupos "dieta inadequada" e "dieta que requer modificações", foi observado menor média de idade no grupo "dieta inadequada" (37,6±14,8 versus 47,4±10,5 anos, P=0,02). Verificou-se que 44,2% dos pacientes estavam acima do peso (índice de massa corporal (IMC) ≥25 kg/m²) e possuíam CC aumentada (mulheres: CC ≥80 cm e homens: CC ≥94 cm). Foi encontrada correlação positiva entre a pontuação final do IQD-R e o IMC (P=0,046; r=0,346). CONCLUSÃO: Os pacientes com DC em remissão clínica e em uso de infliximabe não estão adotando dieta com qualidade considerada saudável o que aponta a necessidade de abordagem nutricional individualizada.

Abstract As the treatment of infectious and parasitic diseases improved, the prevalence of these conditions declined. However, with the expansion of the use of immunobiologicals, opportunistic infections have emerged, especially under atypical presentations. The present study reports the case of a patient treated with infliximab for Crohn's disease, who presented diarrhea, weight loss, abdominal pain, fever, and subcutaneous erythematous nodules that evolved with spontaneous fluctuation and ulceration. With the finding of alcohol-resistant bacilli and Mycobacterium tuberculosis DNA in a cutaneous fragment, through polymerase chain reaction, the diagnosis of gummatous tuberculosis was confirmed, probably secondary to hematogenous dissemination from an intestinal focus.

ABSTRACT BACKGROUND: The rates of postoperative endoscopic recurrence (PER) in patients with Crohn's disease (CD) are consistent. Anti-TNF therapy has been increasingly used in the postoperative setting, despite the lack of robust data in the literature on the measurement of trough levels and consequences of their use. OBJECTIVE: The aim of this review was to assess trough levels of infliximab (IFX) in CD patients after ileocolonic resections in correlation with the presence of PER. METHODS: We searched for studies that evaluated trough levels of IFX in patients with CD, who underwent ileocaecal resections, and correlated them with the presence of PER. We used MEDLINE through PubMed and CENTRAL Cochrane library databases, and after matching the inclusion criteria, the studies were methodologically evaluated with qualitative analysis of the data. RESULTS: A total of 155 studies were initially identified in the databases search and only four matched the inclusion criteria. They comprised one prospective cohort study, one randomized controlled trial and two retrospective cohort studies, the last one performed in pediatric patients. This evidence suggested the correlation of PER with low trough levels of IFX and the presence of antibodies to the drug. The quality of the evidence generated varied from very low to high, due to the heterogeneity found between the studies and the risks of bias that were identified. CONCLUSION: Low levels of IFX and the presence of antibodies to the drug were directly associated with increased PER rates in patients with CD, who underwent ileocolonic resections. Controlled and randomized clinical trials with adequate methodological quality are warranted to confirm the conclusions from this systematic review.


RESUMO CONTEXTO: As taxas de recorrência endoscópica pós-operatória em pacientes com doença de Crohn (DC) são significativas. A terapia anti-TNF é cada vez mais usada no cenário pós-operatório, apesar da escassez de dados na literatura sobre dosagem de níveis séricos e anticorpos da droga. OBJETIVO: Realizou-se uma revisão sistemática com o intuito de se avaliar níveis séricos de infliximabe (IFX) em pacientes com DC submetidos a ileocolectomia e correlacionar com a presença ou não de recorrência endoscópica da doença. MÉTODOS: Buscou-se por estudos que avaliaram o nível sérico do IFX em pacientes com DC, submetidos a ileocolectomias, correlacionando-os à presença de recorrência endoscópica pós-operatória. Utilizou-se as bases de dados MEDLINE via PubMed e CENTRAL Cochrane Library, e após atingirem os critérios de inclusão, os estudos foram avaliados metodologicamente e foi realizada análise qualitativa dos dados. RESULTADOS: Um total de 155 estudos foram identificados e apenas quatro atingiram os critérios de inclusão. Um era estudo de coorte prospectivo, o segundo era um ensaio clínico randomizado e dois eram estudos de coortes retrospectivas, sendo o último exclusivamente em pacientes pediátricos. As evidências encontradas tendem a confirmar a correlação da recorrência endoscópica a baixos níveis séricos de IFX e presença de anticorpos anti-droga. A qualidade da evidência gerada variou de muito baixa a alta, devido à heterogeneidade encontrada entre os estudos e o risco de viés identificado. CONCLUSÃO: Baixos níveis séricos do IFX e presença de anticorpos contra a droga estão associados a probabilidade aumentada de recorrência endoscópica pós-operatória nos pacientes com DC submetidos a ileocolectomias. Ensaios clínicos controlados e randomizados com adequada qualidade metodológica são necessários para confirmar as conclusões desta revisão.

La terapia con fármacos antagonistas del factor de necrosis tumoral alfa ha sido beneficiosa en el tratamiento de varias enfermedades como las del tejido conectivo e inflamatorias del intestino, pero no está exenta de riesgos. Las principales complicaciones de estas drogas inmunosupresoras son las infecciones, y la tuberculosis pulmonar es una de las principales afecciones, que se pueden observar en los pacientes con este tipo de tratamiento.Se presentó una mujer de 31 años, atendida en el Hospital Clínico Quirúrgico Hermanos Ameijeiras, La Habana, Cuba, con antecedentes de colitis ulcerativa, que hace 3 meses recibe terapia con Infliximab. Acude al hospital por referir 4 días previos al ingreso, fiebre de 390 C dos veces al día, acompañándose de cefalea, pérdida del apetito y dolor en la región perineal. Se le realizó radiografía de tórax, donde se describe radiopacidad heterogénea que va desde el cuerno superior del hilio derecho hasta planos axilares, en la tomografía axial de tórax reportan consolidación en segmento anterior del lóbulo superior derecho con presencia de broncograma aéreo y en el lavado bronquial microbiológico para bacilos ácido-alcohol resistentes se informó codificación 8, positivo a Mycobacterium tuberculosis. El diagnóstico preciso de tuberculosis relacionada con el uso de fármacos antagonistas del factor de necrosis tumoral alfa requiere un alto índice de sospecha y una investigación detallada. Existe un alto grado de complejidad diagnóstica, por la existencia de un amplio espectro clínico y la necesidad de excluir otras enfermedades.


Tumor necrosis factor alpha antagonist drug therapy has been beneficial in the treatment of several diseases such as connective tissue and inflammatory bowel diseases, but it is not without risks. The main complications of these immunosuppressive drugs are infections, and pulmonary tuberculosis is one of the main conditions, which can be observed in patients with this type of treatment. A 31-year-old woman, treated at the Hermanos Ameijeiras Clinical Surgical Hospital, Havana, Cuba, with a history of ulcerative colitis, who has been receiving Infliximab therapy for 3 months, presented. He went to the hospital for referring 4 days prior to admission, a fever of 390 C twice a day, accompanied by headache, loss of appetite and pain in the perineal region. A chest X-ray was performed, which described heterogeneous radiopacity that goes from the upper horn of the right hilum to axillary planes, in the chest axial tomography they report consolidation in the anterior segment of the right upper lobe with the presence of air bronchogram and in the bronchial lavage microbiological for acid-fast bacilli coding 8, positive for mycobacterium tuberculosis was reported. Accurate diagnosis of tuberculosis related to the use of tumor necrosis factor alpha antagonist drugs requires a high index of suspicion and detailed investigation. There is a high degree of diagnostic complexity, due to the existence of a wide clinical spectrum and the need to exclude other diseases.

RESUMO - RACIONAL: A etiopatogenia da colite por desuso (DC) ainda não foi totalmente elucidada. As principais teorias consideram que a doença pode estar relacionada ao aumento de bactérias anaeróbias, falta de suprimento de ácidos graxos de cadeia curta (AGCC) e distúrbios imunológicos que se desenvolvem em segmentos colorretais desprovidos de trânsito fecal. OBJETIVO: Verificar se a aplicação de infliximabe modifica o conteúdo tecidual das proteínas E-caderina e claudina-3 no epitélio cólico de ratos sem trânsito intestinal. MÉTODOS: Vinte dois ratos foram submetidos a derivação do trânsito intestinal pelo procedimento de Hartmann. Eles permaneceram com o ostoma por 12 semanas para permitir o desenvolvimento da colite de exclusão. Em seguida, foram divididos em três grupos experimentais: seis animais receberam 2,0 ml de solução salina/semana, oito infliximabe na dose de 5 mg/Kg/semana e, os demais, infliximabe na dose de 10 mg/Kg/semana por 5 semanas consecutivas. Em seguida, os animais foram eutanasiados e os segmentos cólicos com e sem trânsito intestinal foram removidos. A colite por desuso foi diagnosticada pelas alterações histológicas definidas por uma escala previamente validada. Expressão tecidual de E-caderina e claudina-3 foi avaliada por imuno-histoquímica, e o conteúdo tecidual de ambas as proteínas foi quantificado por análise de imagem assistida por computador. RESULTADOS: Segmentos cólicos exclusos de trânsito fecal apresentaram maior grau de inflamação do que os expostos ao trânsito fecal. Inflamação foi menor nos animais tratados com infliximabe, independente da dose utilizada. Níveis de E-caderina e claudina-3 estavam reduzidos no cólon excluso. O tratamento com infliximabe aumentou os níveis das proteínas em segmentos do cólon sem trânsito intestinal, principalmente nos animais que receberam a dose de 10mg/kg/semana. CONCLUSÃO: Infliximabe reduz inflamação nos segmentos do cólon excluso e aumenta o conteúdo tecidual de E-caderina e claudina-3, especialmente na concentração de 10mg/kg/semana.


ABSTRACT - BACKGROUND: The etiopathogenesis of disuse colitis (DC) has not yet been fully elucidated. The main theories consider that the disease may be related to an increase in anaerobic bacteria, the lack of short-chain fatty acid (SCFA) supply, and immunological disorders that develop in the colorectal segments devoid of fecal transit. AIM: The aim of this study was to verify whether the application of infliximab modifies the tissue content of E-cadherin and claudin-3 proteins in colonic epithelium of rats devoid of intestinal transit. METHODS: A total of 22 rats underwent intestinal transit bypass using Hartmann's procedure. They remained with the shunt for 12 weeks to allow the development of DC. Later, they were divided into three experimental groups: six animals received 2.0 mL saline solution/week, eight received infliximab at a dose of 5 mg/kg/week, and eight received infliximab at a dose of 10 mg/kg/week for 5 consecutive weeks. At the end of this period, the animals were euthanized, and the colonic segments with and without intestinal transit were removed. DC was diagnosed based on the histological changes defined by a previously validated scale. The tissue expression of E-cadherin and claudin-3 was assessed by immunohistochemistry, and the tissue content of both proteins was quantified by computer-aided image analysis. RESULTS: The colonic segments excluded from fecal transit showed a higher degree of inflammation than those exposed to fecal transit. The degree of inflammation was lower in animals treated with infliximab, regardless of the dose used. The levels of E-cadherin and claudin-3 were reduced in the excluded colon. Treating animals with infliximab increased the levels of both proteins in the colonic segments without intestinal transit, especially in animals receiving a dose of 10 mg/kg/week. CONCLUSION: Infliximab therapy reduces inflammation in the colonic segments excluded from intestinal transit and increases the tissue content of E-cadherin and claudin-3 proteins, especially when used at a concentration of 10 mg/kg/week.

ABSTRACT Purpose Quantify the tissue content of metalloproteinase-9 (MMP-9) and collagen in colic mucosa with and without intestinal transit after infliximab administration in rats subjected to Hartmann's surgery. Methods Twenty-two rats underwent colon diversion by Hartmann's surgery. Animals were maintained with intestinal bypass for 12 weeks to induce development of diversion colitis (DC). Afterwards, animals were divided into three groups: first group received subcutaneous application of saline solution (SS) 0.9%, while the remaining two groups received infliximab subcutaneously at doses of 5 or 10 mg·kg-1·week-1 for five consecutive weeks. After the intervention, animals were sacrificed, removing the segments with and without intestinal transit. Diversion colitis was diagnosed by histological study, and its intensity was determined by a validated inflammatory scale. Tissue expression of MMP-9 was assessed byimmunohistochemistry, while total collagen was assessed by histochemistry. Tissue content of both was measuredby computerized morphometry. Results Colon segments without intestinal transit had a higher degree of inflammation, which improved in animals treated with infliximab. Collagen content was always lower in those without intestinal transit. There was an increase in the collagen content in the colon without transit in animals treated with infliximab, primarily at a dose of 10 mg·kg-1·week-1. There was an increase in the content of MMP-9 in the colon without fecal transit, and a reduction was observed in animals treated with infliximab, regardless of the dose used. Conclusions Application of infliximab reduces inflammation, increases the total collagen content and decreases the content of MMP-9 in the colon without intestinal transit.

ABSTRACT

Objetivo: descrever as evidências disponíveis na literatura científica sobre eficácia e segurança do rituximabe comparado a diferentes tratamentos. Materiais e métodos: é uma revisão rápida de evidências científicas para tomada de decisão informada por evidências em políticas e práticas de saúde. Conclusão: o Rituximabe tem eficácia e segurança similares à da Ciclofosfamida, para terapia de indução de remissão e para manutenção da remissão e, para pacientes com doença recidivante, o Rituximabe é mais eficaz que a Ciclofosfamida para manter a remissão. Para terapia de manutenção, Rituximabe é mais eficaz que Azatioprina, com perfil de segurança similar. Diferentes regimes de dosagem do Rituximabe tem eficácia e segurança similar para terapia de manutenção. O Infliximabe parece ser superior ao Rituximabe nos desfechos de eficácia (indução e manutenção da remissão).


Objective: to describe the evidence available in the scientific literature on the efficacy and safety of rituximab compared to different treatments. Materials and Methods: is a rapid review of scientific evidence for evidence-informed decision making in health policy and practice. Conclusion: Rituximab has similar efficacy and safety to Cyclophosphamide, for remission induction therapy and for maintenance of remission, and for patients with relapsing disease, Rituximab is more effective than Cyclophosphamide in maintaining remission. For maintenance therapy, Rituximab is more effective than Azathioprine, with a similar safety profile. Different dosing regimens of Rituximab have similar efficacy and safety for maintenance therapy. Infliximab appears to be superior to Rituximab in efficacy outcomes (induction and maintenance of remission).

BACKGROUND@#Psoriasis is a common, chronic, immune-mediated inflammatory skin disease with increased epidermal proliferation. The objective of this review was to systematically identify the evidence and perform a network meta-analysis (NMA) to estimate the relative efficacy of secukinumab (SEC) against adalimumab (ADA) and infliximab (INF) for the treatment of moderate-to-severe plaque psoriasis.@*METHODS@#A systematic literature review (SLR) was conducted according to a pre-specified protocol to identify relevant studies. Initially, the databases were searched from database inception till June 2013, and the SLR was updated in April 2020. The eligibility criteria included adult patients (≥18 years old) with moderate-to-severe plaque psoriasis, and the SLR included randomized controlled trials (RCTs). The comparators of interest were SEC, ADA, INF, and placebo (PLA), while outcomes of interest were Psoriasis Area and Severity Index (PASI) (50, 75, and 90) at weeks 12, 16, and 24. A Bayesian NMA for PASI was utilized with a framework that evaluated the probability of PASI responses in different categories of PASI thresholds within a single model.@*RESULTS@#A total of 23 RCTs that assessed the efficacy of SEC, ADA, and INF in patients with moderate-to-severe plaque psoriasis were identified. At 12 weeks, SEC was associated with a significantly better response compared with PLA and ADA for PASI 75 and 90, while response results were comparable against INF. At 12 weeks, risk ratio (95% confidence interval) derived from NMA for SEC vs. ADA and INF for PASI 75 was 1.35 (1.19, 1.57) and 1.01 (0.90, 1.18), respectively. At the 16-week and 24-week time interval, SEC was significantly better than PLA, ADA, and INF for PASI 75 and 90.@*CONCLUSION@#Efficacy of SEC in the treatment of patient populations with moderate-to-severe plaque psoriasis is well demonstrated through NMA.

ABSTRACT

As doenças inflamatórias intestinais (DIIs) são enfermidades crônicas desencadeadas por grave inflamação no trato gastrointestinal. Entre os mediadores imunes envolvidos na patogênese das DIIs, o fator de necrose tumoral alfa (TNF-α) e sugerido como citocina primordial. Assim, ferramentas de inativação da via do TNF-α, como o infliximabe (IFX), tem sido amplamente aplicadas no tratamento destas doenças. Embora o IFX seja eficaz na indução/manutenção de remissão das DIIs, ainda há relatos de efeitos adversos ou pacientes refratários a imunoterapia. Por esse motivo, e emergente a necessidade de identificar biomarcadores associados ao sucesso das terapias biológicas. Estudos prévios em pacientes com Doença de Crohn (DC) mostraram que a expressão da proteína anti-inflamatória anexina A1 (AnxA1) sistêmica e na mucosa intestinal e aumentada após IFX e correlacionada com a melhora da qualidade de vida. A AnxA1 e o seu peptídeo N-terminal, Ac2-26, desempenham suas funções na resolução da inflamação via receptores para peptídeo formilado (FPRs). No presente estudo, nós investigamos de que maneira os FPRs e a AnxA1 participam dos mecanismos do IFX. No modelo de colite experimental induzida por dextran sulfato de sódio (DSS) em camundongos selvagens (WT) e deficientes para AnxA1 endógena (AnxA1-/-), o IFX atenuou as manifestações clínicas da doença apenas em animais WT. O bloqueio dos FPRs com o antagonista Boc-2 reverteu a melhora observada nos animais tratados, enquanto a ausência da AnxA1 endógena revogou completamente a eficácia do tratamento. Ainda, a resposta inflamatória da colite foi exacerbada nos animais AnxA1-/- após IFX, que apresentaram redução de células T regulatórias e aumento da MMP9 no colon, encurtamento do intestino grosso, ausência de melhora histológica e mortalidade de 50%. Nos animais WT, por sua vez, o bloqueio dos FPRs impediu a melhora clínica e a regeneração das criptas mucosas, com desorganização da ß-actina e da borda em escova. Nas células epiteliais do intestino da linhagem Caco-2 estimuladas por TNF-α in vitro, confirmamos que os efeitos protetivos do IFX nas junções celulares são perdidos após bloqueio do FPR1 e do FPR2, comprometendo a integridade desta barreira. No colon, o IFX induziu a expressão e secreção da AnxA1 em células da lamina própria após colite, e essa secreção foi dose-dependente em células da lamina própria tratadas ex vivo, demonstrando que a secreção da AnxA1 por células do tecido conjuntivo pode constituir um dos mecanismos resolutivos desta terapia. Em humanos, o tratamento com IFX na DC não modificou as expressões de FPR1 e FPR2 nos leucócitos circulantes ou da AnxA1 plasmática, não permitindo a diferenciação de pacientes responsivos ou não. No entanto, a AnxA1 tecidual estava aumentada em pacientes que respondem ao IFX, e os níveis de AnxA1 e FPR1 foram negativamente correlacionados a remissão histológica. Por fim, análises de bioinformática revelaram expressões diferenciais dos mRNAs de FPR1, FPR2 e AnxA1 no colon entre pacientes remissivos ou refratários mesmo antes do início das infusões, mas esse mesmo padrão não foi observado nos PBMCs do sangue. Em conclusão, sugerimos que a indução da AnxA1 pode ser um dos mecanismos de resolução do IFX, e que é complementado pela ativação de FPRs. Ainda, estes marcadores podem apresentar valor preditivo da eficácia do IFX, contribuindo para o alcance da remissão da DC de maneira mais rápida e permanente


Inflammatory bowel diseases (IBDs) are chronic debilitating illnesses triggered by severe inflammation of the gastrointestinal tract. The tumor necrosis factor alpha (TNF-α) is pointed out as a primordial mediator of IBD pathogenesis; thus, inactivating tools targeting this cytokine have been widely used to treat these diseases. Although IFX is very efficient in inducing/maintaining remission in patients with IBD, side effects and unresponsiveness are still reported, emerging the need for the identification of biomarkers linked to therapeutical efficacy. In the Crohns disease (CD), systemic and tissue expressions of the anti-inflammatory protein, annexin A1 (AnxA1), are increased after IFX treatment and correlate with life quality improvement according to previous reports. AnxA1 and its N-terminal peptide, Ac2-26, act via formyl peptide receptors (FPRs); therefore, the present investigation aimed to understand how FPRs and AnxA1 participate in IFX mechanisms. In the experimental colitis model induced by dextran sulphate sodium (DSS) in wild-type (WT) and AnxA1-deficient mice (AnxA1-/-), IFX attenuated the clinical manifestations only in the WT group. FPRs blockade using the antagonist, Boc-2, impaired IFX effects in WT mice, while AnxA1 absence completely abrogated its efficacy. Furthermore, the inflammatory response was exacerbated after IFX in AnxA1-/- mice, with reduced T regulatory cells, increased tissue MMP9, large intestine shortening, lack of histological remission and 50% mortality. In WT mice, FPR blockade reverted the clinical recovery and mucosal crypts regeneration, with b-actina and brush border disorganization. Using the intestinal epithelial cell line Caco-2 stimulated with TNF-α in vitro, we confirmed that IFX protective effects on tight junctions are lost after FPR1 and FPR2 blockade, compromising the barrier integrity. In the colonic tissue, the expression and secretion of AnxA1 were induced by IFX after colitis. This secretion was shown to be dose-dependent in cells from the intestinal lamina propria treated ex vivo, demonstrating that secreted AnxA1 could constitute one of the resolutive mechanisms of that therapy. In humans with CD, IFX did not modify the expressions of FPR1 and FPR2 in circulating leukocytes or the plasma AnxA1 levels, not differentiating patients responsive or not. However, tissue AnxA1 expression was augmented in responsive patients, and AnxA1/FPR1 levels were negatively correlated with histological remission. Finally, bioinformatic analyses revealed differential expression of FPR1, FPR2 and AnxA1 mRNAs in the colon among remittent or refractory patients even before the beginning of infusions, which was not observed for samples of blood PBCM. In conclusion, we suggest that inducing tissue AnxA1 might be one of the resolution mechanisms of IFX, which is complemented by the activation of FPRs. Moreover, these markers could present predictive value of IFX efficacy, contributing to reaching an early and more permanent remission in IBD

ABSTRACT BACKGROUND: Inflammatory bowel diseases (IBD) are chronic inflammatory affections of recurrent nature whose incidence and prevalence rates have increased, including in Brazil. In long term, they are responsible for structural damage that impacts quality of life, morbidity and mortality of patients. OBJECTIVE: To describe the profile of physicians who treat IBD patients as well as the characteristics of IBD care, unmet demands and difficulties. METHODS: A questionnaire containing 17 items was prepared and sent to 286 physicians from 101 Brazilian cities across 21 states and the Federal District, selected from the register of the State Commission of the "Study Group of Inflammatory Bowel Disease of Brazil" (GEDIIB). RESULTS: The majority of the physicians who answered the questionnaire were gastroenterologists and colorectal surgeons. More than 60% had up to 20 years of experience in the specialty and 53.14% worked at three or more locations. Difficulties in accessing or releasing medicines were evident in this questionnaire, as was referrals to allied healthy professionals working in IBD-related fields. More than 75% of physicians reported difficulties in performing double-balloon enteroscopy and capsule endoscopy, and 67.8% reported difficulties in measuring calprotectin. With regard to the number of patients seen by each physician, it was shown that patients do not concentrate under the responsibility of few doctors. Infliximab and adalimumab were the most commonly used biological medicines and there was a higher prescription of 5-ASA derivatives for ulcerative colitis than for Crohn's disease. Steroids were prescribed to a smaller proportion of patients in both diseases. The topics "biological therapy failure" and "new drugs" were reported as those with higher priority for discussion in medical congresses. In relation to possible differences among the country's regions, physicians from the North region reported greater difficulty in accessing complementary exams while those from the Northeast region indicated greater difficulty in accessing or releasing medicines. CONCLUSION: The data obtained through this study demonstrate the profile of specialized medical care in IBD and are a useful tool for the implementation of government policies and for the Brazilian society as a whole.


RESUMO CONTEXTO: As doenças inflamatórias intestinais (DII) são afecções inflamatórias crônicas de caráter recorrente, cujas taxas de incidência e prevalência têm aumentado, inclusive no Brasil. A longo prazo, são responsáveis por danos estruturais que impactam na qualidade de vida, morbidade e mortalidade dos pacientes. OBJETIVO: Avaliar o perfil dos médicos que atendem pacientes com DII, assim como as características deste atendimento, demandas não atendidas e dificuldades. MÉTODOS: Um questionário contendo 17 variáveis foi elaborado e enviado para médicos, selecionados a partir do cadastro da Comissão das Estaduais do Grupo de Estudos da Doença Inflamatória Intestinal do Brasil (GEDIIB), totalizando 286 médicos de 101 cidades brasileiras distribuídas por 21 estados e Distrito Federal. RESULTADOS: A maioria dos médicos que respondeu o questionário foram Gastroenterologistas e Coloproctologistas. Mais de 60% tinham até 20 anos de atuação na especialidade e 53,14% trabalhavam em três locais ou mais. A dificuldade no acesso ou liberação de medicamentos ficou evidenciada neste questionário, assim como a dificuldade no encaminhamento para profissionais não médicos que atuam em DII. Mais de 75% dos médicos relataram dificuldades para realização de enteroscopia por duplo balão e cápsula endoscópica, e 67,8% para realização da calprotectina. Em relação ao número de pacientes atendidos por cada médico, foi evidenciado que não há uma concentração de pacientes sob a responsabilidade de poucos médicos. O infliximabe e o adalimumabe foram os biológicos mais utilizados e ficou evidenciada prescrição maior de derivados de 5-ASA para retocolite ulcerativa quando comparada à doença de Crohn. Os corticoides foram prescritos para uma parcela menor de pacientes em ambas doenças. Os temas "falha a terapia biológica" e "novas drogas" foram referidos como aqueles com maior prioridade para discussão em eventos científicos. Em relação às possíveis diferenças entre cada região e o restante do país, os médicos da região Norte relataram maior dificuldade no acesso a exames complementares e os médicos da região Nordeste, maior dificuldade no acesso ou liberação de medicamentos. CONCLUSÃO: Os dados obtidos por meio deste estudo mostram o perfil do atendimento médico especializado em DII e podem se constituir em ferramenta útil para para elaboração de políticas governamentais e para sociedade brasileira como um todo.
